1. Booking Conditions

1.1
The £250 consultation fee will be deducted from the total cost of the treatment. If the treatment does not take place the £250 consultation fee does not become refundable. The treatment is to take place on the same day as a initial consultation in order to qualify for the £250 deduction.

2. Cancellation Charges

2.1
If the consultation/treatment appointment is cancelled and not rescheduled the following charges will be applied, please refer to point 2.3 and 2.4:

2.2
If Dr Nick Milojevic cancels your appointment, a full refund will be provided.

2.3
Cancellation charges will not apply if consultation/treatment appointment is re-arranged at least 48 hours prior to your appointment. Please note we cannot guarantee you will be able to re-arrange your appointment for the same week. Availability will be confirmed at the time of rescheduling your appointment. Appointment amendments will only be amended once free of charge. Please refer to point 2.4

2.4
If your appointment is rescheduled less than 48 hours you will incur an £250 cancellation charge. This will be taken before re-scheduling your appointment. We cannot guarantee you will be able to re-arrange your appointment for the same week. Availability will be confirmed at the time of rescheduling your appointment.

3. Payment Conditions

3.1
New patients may be asked to pay for treatment prior to the treatment taking place should treatment be opted for on the day of the consultation.

3.2
Full payment for consultation, treatment or products is required, credit will not be given.

3.3
Major credit cards, other than American Express and Debit Cards are accepted.

4. Treatment Conditions

4.1
All patients are to complete a medical history form (please refer to point 4.2), read through the product information and sign the consent form as instructed prior and every time a treatment takes place. All patients will be sent the aftercare instructions via e-mail along with their appointment confirmation and given a copy on the day of their treatment.

4.2
All past patients will be asked to sign a declaration confirming that their medical history has not changed since the day of completion. It is the patient’s responsibility to inform Dr Milojevic of any changes to their medical history, however insignificant it may seem. Dr Milojevic reserves the right to ask for re-completion of a patient’s medical history at any time.

5. Product Purchases

5.1
OBAGI products are available to order from Dr Nick Milojevic, following a consultation/discussion. Please ask for a full list of the products which are available to order. If you would like to place an order, please e-mail info@milojevic.hr listing the products you would like. Please see section 5.3.

5.2
If your treatment is the OBAGI 6-18 week course programme and you require further products, please contact info@milojevic.hr listing the products you require. You will be sent confirmation of the cost of each product once your request has been confirmed by Dr Milojevic. Once written confirmation from you has been received, full payment will be required and is to be paid via the telephone (please refer to point 3.2). You will be charged for postage and packaging, items will be sent recorded or special delivery. Please note until confirmation and payment has been received, the products will not be ordered. You will be sent an e-mail confirming payment has been made and your products have been ordered. Please note the products are non-refundable for hygiene reasons.

5.3
Confirmation of the cost of each product will be sent to you via e-mail, once written confirmation from you has been received, full payment is to be made via the telephone; please refer to point 3.2. You will be charged for postage and packaging, items will be sent recorded or special delivery. Please note until confirmation and payment has been received, the products will not be ordered. You will be sent an e-mail confirming payment has been made and your products have been ordered. Please note the products are non-refundable for hygiene reasons.

6. Referrals to Poliklinika Milojevic

6.1
Milo Clinic acts as a referral centre only, and all responsibilities for the patient, the patient’s procedure and the aftercare are that of Poliklinika Milojevic once the referral has been made.

6.2
Milo clinic will provide assistance to Poliklinika Milojevic in the aftercare of the patient, once they return to the UK, but will only act as the eyes and ears of Poliklinika Milojevic, and all advice and treatment thereafter will be under strict instructions of Poliklinika Milojevic in Croatia.

6.3
Milo clinic does not benefit financially at all from the operations carried out in Croatia.

6.4
Milo clinic and Poliklinika Milojevic are not connected legally or financially, and act as referral clinics to each other only.
